Position Summary Buffalo Homecare is seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a Field Nurse. In this role, you will travel throughout Western New York conducting client assessments, supervisory visits, nursing evaluations, and care coordination activities to ensure clients receive the highest quality of care in their homes. This position offers the opportunity to work independently while making a meaningful impact on the lives of the individuals and families we serve.

Company vehicle Company cell phone Company laptop Responsibilities Conduct initial nursing assessments and periodic reassessments. Perform supervisory visits for home care aides. Develop and update individualized plans of care. Coordinate care with physicians, caregivers, and interdisciplinary team members. Educate clients and families regarding health conditions, medications, and safety measures. Travel to client homes and community settings throughout the service area. Maintain accurate and timely documentation in compliance with agency, state, and federal requirements. Participate in case conferences and interdisciplinary meetings. Ensure quality assurance standards and regulatory compliance are maintained. Participate in ongoing training and professional development. Qualifications Current and unrestricted Registered Nurse (RN) license in New York State. Minimum of 3 years of nursing experience required. Strong clinical assessment and care coordination skills. Excellent communication, organizational, and interpersonal skills. Ability to work independently and manage a flexible schedule. Experience with Electronic Medical Records (EMR) systems preferred. Valid New York State driver's license required. Basic Life Support (BLS) certification required.
