Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage plan participants Assess both prior and current medical history, chronic conditions, and medication regimens Deliver individualized education to patients about their medical conditions and prescribed medications Proactively identify care gaps and provide actionable recommendations to address these gaps No requirement to create treatment plans or prescribe medications, focusing solely on evaluation and education

Qualifications:

Current Nurse Practitioner license in New York Valid BLS (CPR) certification National board certification through AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred MSN from an accredited nursing program Benefit from compensation between $80-$90 per evaluation, plus a mileage stipend and reimbursement for administrative time, making this an excellent way to supplement your income with meaningful work.
