This contract role focuses on providing high-quality care in a long-term care setting, supporting residents in their daily living activities, and fostering their independence and well-being.
Key Qualifications:
Current, unencumbered RN license Experience in rehab and/or long-term care settings Strong assessment, communication, and documentation skills Compassionate approach to patient care Ability to collaborate effectively with interdisciplinary teams Familiarity with electronic health records (EHR) and care plans
Key Responsibilities:
Provide direct nursing care to residents, following individualized care plans Assess, monitor, and document residents' health condition and progress Administer medications and treatments as ordered Support rehabilitation programs and encourage patient participation Communicate with families, physicians, and team members to coordinate care Respond to emergencies promptly and appropriately Ensure compliance with facility policies and state/federal regulations
