Category Nursing - RN-Rehab/LTC Job Type Contract Advance your nursing career by supporting patients in a long-term care and rehabilitation environment. This contract position is well-suited for a dedicated Registered Nurse who thrives on providing compassionate care, individualized support, and expert clinical judgment in a rehabilitation or long-term care setting.
Key Responsibilities:
Assess and monitor residents' health status, create care plans, and provide direct nursing care tailored to each patient's unique needs Administer medications and treatments in accordance with policy, physician orders, and best nursing practices Collaborate with an interdisciplinary team, including physicians, therapists, and social workers, to optimize each patient's recovery and quality of life Provide health education and emotional support to residents and their families, empowering them with knowledge for optimum wellbeing Accurately document care, changes in condition, interventions, and outcomes using electronic health records and standard documentation protocols Maintain a safe, clean, and therapeutic environment while ensuring compliance with all regulatory and facility guidelines
Qualifications & Desired Experience:
Valid RN license in New York State Previous experience in rehab, skilled nursing, or long-term care strongly preferred Strong assessment, critical thinking, and communication abilities Ability to work effectively within a team and independently, prioritizing patient care Current Basic Life Support (BLS) certification required
