Job Description

LICENSED PRACTICAL NURSE
(LPN)
Summary:
The LPN is responsible for the overall case management, and medication supervision of the residents, including personal care services in the Assisted Living Community.. Responsibilities include, but not limited to accurate transcribing orders, working with residents doctors and pharmacies to ensure the MAR's are up to date at all times, medication ordering and procurement, hiring and training of the med techs and personal care aides; annual review and ongoing coaching and counseling of the staff members: completion of case management notes, staffing schedule and submitting payroll hours. Ensure that the universal worker concept is implemented. As the leader of the resident care services is expected to develop and maintain a positive atmosphere for residents, families, visitors and staff. This candidate must be approved by the Department of Health.
Qualifications:
LPN with Two (2) years experience in long term care or assisted living College Degree Available to work a Tuesday to Saturday schedule Supervisory and management experience Current state license as an LPN in New York Ability to handle multiple priorities Able to direct the work of others Excellent verbal and written communication skills Has good organizational and time management skills Experience with seniors preferred Strong verbal and written communication skills required' Able to make responsible choices and decisions and act in the a residents best interest Reports to: Executive Director Supervisory/Exempt Recruits, hires, supervises, trains and coaches all personal care workers in the home • Completes a 90-day performance reviews and all subsequent annual reviews• Areas of Responsibility Oversight of Medication Management Services Procurement of medications and medication refills Hiring and training Budget and staffing Administrative Leadership Resident Case Management work with RN to reconcile doctors orders ensure that there are no expired medications Completed initial, quarterly and annual case management notes Ensure appropriate staffing scheduling of med techs Communicates regularly with families , pharmacies and physicians On-call 24 hours for staff call-outs, scheduling and questions Hiring & Training Participate in the interviewing and hiring process. Participate in the ongoing structured orientation program for all new personal care aids employees. Conduct a 90 day performance review and make recommendations for continued employment Provide regular, on-going training as needed. Budget and Staffing Maintain budgetary guidelines for daily staffing hours Complete variance reports as requested by Executive Director Administrative Provide daily coaching and support to personal care aids Lead by example, being visible, involved and accessible Must take on call responsibilities and manage a staff that works 24 hours a day Leadership Complete staffing schedule in a timely manner according to budget guidelines Complete and submit staffing hours for payroll Complete timely performance reviews Benefits Medical,Dental,Vision
