Experienced Nurse Practitioners are needed to perform home health risk assessments on Medicare Advantage plan members residing throughout Jefferson County. This per diem position offers flexibility, autonomy, and a meaningful way to impact patient care in the local community. Work is scheduled in various areas within the county, including Le Ray, Watertown, Adams, and Fort Drum.

Key Responsibilities:

Conduct annual home health risk assessments in client homes for Medicare Advantage members Assess and review prior and current medical history, conditions, and medication use Provide education to members on health conditions and prescribed medications Identify gaps in care and make appropriate recommendations for follow-up or resolution No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active and unrestricted Nurse Practitioner license in New York State Current BLS CPR certification Board Certified through AANP or ANCC Minimum of 6 months of Nurse Practitioner experience preferred MSN degree from an accredited nursing program This opportunity offers a per evaluation compensation ranging from $80 to $90, supplemented by mileage reimbursement and administrative pay, ensuring your expertise and effort are well supported.
