$140,000 to $160,000 based on experience

Urban Health Plan is looking for AGNPs to provide primary care services as part of our primary care team. The AGNP will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The AGNP will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team.
