Admit and reassess home health patients Coordinate and supervise Home Health Aides Conduct home care visits and reassess regularly Implement care plans and document progress Educate patients and caregivers

Requirements:

NYS RN License Positive collaboration skills Strong communication and time-management Ability to travel daily
