Schedule Flexible shifts available Monday through Sunday Create your own schedule Minimum weekly or bi-weekly hourly commitment Key Responsibilities Perform annual and post-discharge home health risk assessments for Medicare Advantage members Assess patients prior/current medical histories, medication use, and chronic conditions Educate patients regarding health conditions and proper medication management Identify care gaps and offer actionable recommendations No need to prescribe medications or develop treatment plans Qualifications Active Nurse Practitioner license in the state of NY MSN degree from an accredited nursing school Board Certification from AANP or ANCC Current CPR certification (BLS) 6 months recent Nurse Practitioner experience preferred Compensation and Perks Earn $80$90 per completed evaluation, with additional mileage and administrative stipends.
