ASAP Schedule:

3-4 days a week

Hours:

8am to 4pm or 9am to 5pm (30 hours per week)

Patient Population:

ID/DD (Ages 30+)

Responsibilities:

Work in conjunction with medical providers to ensure highest quality of integrated healthcare for people supported in the organization Review, modify, and update the individual's plan of nursing care in accordance with medical diagnoses and treatment plan Administer medications, ensuring medication is given as ordered and documented accurately. Monitor effects of all medications and document as necessary Complete all nursing assessments and associated documentation per agency policy Perform physical observation of the individuals and complete required documentation in a timely, accurate manner per compliance regulations Maintain confidentiality of any information relating to the individuals in the program as per HIPPA guidelines Perform other duties, as needed

Qualifications:

Graduate of an accredited School of Nursing NYS Registered Nurse (RN) license Ability to manage multiple priorities and ability to be flexible to meet the needs of the population. Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized

Desired Qualifications:

Bachelor's Degree in Nursing Experience working with the differently abled population Knowledge of OPWDD, DOH, or OMH state and federal regulations
