HHA/PCA Hempstead, NY $22.19 - $23.19 an hour - Part-time, Full-time $22.19 - $23.19 an hour - Part-time, Full-time Job Summary Join our compassionate healthcare team as a Home Health Aide (HHA) or Personal Care Assistant (PCA), where your dedication and caregiving skills will make a meaningful difference in the lives of individuals needing support with daily activities and medical care. In this vital role, you will provide personalized assistance in patients' homes or care facilities, ensuring their safety, comfort, and well-being. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor and observe patients' conditions, including vital signs, medication adherence, and overall health status, documenting changes accurately in Electronic Health Record (EHR) systems Support clients with memory care needs, including Alzheimer's and dementia care, ensuring a safe environment through vigilant patient observation Administer medications following medical documentation protocols while maintaining HIPAA compliance to protect patient privacy Assist with medical procedures such as catheter care, hospice support, and working with individuals with developmental disabilities or disabilities Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and safety standards Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being Requirements Prior experience in home health care, nursing homes, assisted living or long-term care settings preferred Knowledge of medical terminology, medication administration, vital signs monitoring, and basic medical documentation practices Certification in CPR is required; additional certifications such as hospice care training or dementia/cognitive impairment training are a plus Ability to work independently while following detailed care plans and protocols Experience working with individuals with disabilities or developmental challenges is highly valued Strong communication skills to effectively collaborate with clients, families, and healthcare teams Valid driver's license and reliable transportation are preferred for home visits Join us if you're passionate about providing compassionate patient care and eager to grow your career in a dynamic healthcare environment.
