Key Responsibilities:

Complete thorough annual home health risk assessments (approx. 45 minutes each) for Medicare Advantage members in their homes Review, evaluate, and document patients prior and current medical history, ongoing conditions, and medication history Provide clear education to patients on their conditions and medications Identify gaps in care, offering focused recommendations to improve patient outcomes Visits may include annual health assessments and post-discharge check-ins Please note: No prescribing or treatment planning is required

Qualifications:

Active Nurse Practitioner License in New York Board Certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Current CPR (BLS) certification At least six months of current NP experience preferred You will receive payment of $80$90 per evaluation, with additional mileage stipend and compensation for administrative time, recognizing the value of your skills and commitment.
