Complete UAS-NY assessments accurately and efficiently. Evaluate patients' medical, functional, and long-term care needs. Develop objective clinical assessments and documentation. Coordinate with physicians, case managers, and interdisciplinary healthcare teams. Help patients gain access to appropriate long-term care services. Maintain accurate and timely electronic documentation.
