- Part-time Job Summary We are seeking a dynamic and compassionate Geriatric Nurse Practitioner to join our dedicated healthcare team.
In this vital role, you will provide comprehensive primary and specialized care to elderly patients, promoting health, managing chronic conditions, and enhancing quality of life. Your expertise will help bridge the gap between medical treatment and personalized senior care, ensuring our patients receive respectful, attentive, and expert healthcare services. Duties Conduct thorough medical assessments, including vital signs, physical examinations, and detailed patient histories tailored to geriatric populations Develop and implement individualized care plans that address complex health issues common in seniors such as dementia, and chronic disease management Prescribe medications and treatments following evidenced based treatment including deprescribing and managing polypharmacy for the older adult Manage acute and chronic conditions by coordinating with multidisciplinary teams across various settings Utilize advanced EMR/EHR systems to document patient encounters accurately and efficiently while maintaining compliance with HIPAA standards Provide health coaching and education to patients and their families on disease prevention, medication adherence, nutrition, and lifestyle modifications Skills Extensive experience working with elderly populations in settings such as internal medicine, palliative care, neurology, and others or settings such as nursing homes, assisted living facilities, or memory care units Proficiency in electronic health record systems including Epic, Cerner, or eClinicalWorks; familiarity with Athenahealth is a plus Knowledge of
ICD-10/ICD-9
coding for accurate diagnosis documentation Skilled in managing complex cases involving acute pain management, infectious disease care, palliative medicine, and behavioral health issues Knowledge of pharmacology related to psychopharmacology and drug regulation (DEA regulations) for safe prescribing practices Excellent patient assessment skills with a strong understanding of physiology and anatomy relevant to geriatrics Our providers have been practicing in our current geriatric practice for over 20 years and we are so excited to create our NEW combined Geriatric, Dementia Care, and Palliative care practice providing an exceptional experience for Geriatric populations.
