Key Responsibilities:

Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members Review each patients prior and current medical and medication history Provide patient education regarding health conditions and prescribed medications Identify and address gaps in care, offering clear recommendations No responsibilities for developing treatment plans or prescribing medications

Qualifications:

Current Nurse Practitioner license in New York Valid Basic Life Support (BLS) CPR certification Board certification from AANP or ANCC Minimum of six months nurse practitioner experience preferred MSN degree from an accredited nursing program This role offers a highly competitive per-evaluation rate ranging from $80 to $90, plus a mileage stipend and compensation for administrative time.
