Job Description
Join a dedicated team at a New York State OASAS Certified Outpatient Substance Use Disorder Treatment Program, committed to providing high-quality, compassionate care to individuals with substance use and co-occurring health needs. This temporary, part-time Nurse / Health Coordinator position offers the opportunity to make a meaningful impact by supporting patient health and wellness during a staff leave of absence. The role is ideal for a Registered Nurse (RN) or Nurse Practitioner (NP) with an interest or background in behavioral health and substance use treatment, seeking a flexible schedule and the chance to collaborate with a multidisciplinary team in a supportive, professional environment. Responsibilities Conduct nursing and medical assessments for new patient admissions in accordance with OASAS regulations and program policies. Review patient medical histories, medications, allergies, and current physical health concerns. Coordinate care related to patients physical health needs with primary care providers, specialists, hospitals, and community healthcare agencies. Monitor and document patient health concerns, communicating relevant information to the interdisciplinary treatment team. Assist with medication coordination and health-related referrals as appropriate. Maintain accurate and timely clinical documentation in compliance with agency, OASAS, and HIPAA requirements. Ensure the provision of education, counseling, risk reduction, and referral services related to communicable diseases, including HIV/AIDS, PrEP, PEP, Tuberculosis, Viral Hepatitis, STIs, and other communicable diseases as required by OASAS standards. Provide patients with educational materials and referrals for testing, prevention, and treatment services. Facilitate a weekly Health & Wellness Group for patients. Coordinate follow-up care and referrals related to communicable disease screening and treatment. Support health promotion and wellness initiatives within the program. Collaborate with clinical staff, medical providers, and community healthcare resources to support integrated patient care. Qualifications Current New York State Registered Nurse (RN) license in good standing, or Current New York State Nurse Practitioner (NP) license and certification in good standing Experience in substance use disorder treatment, behavioral health, community health, or outpatient settings preferred Knowledge of New York State OASAS regulations and standards preferred Experience providing communicable disease education and coordination of care preferred Strong organizational, communication, and interdisciplinary collaboration skills Ability to maintain professional boundaries and work effectively with diverse patient populations Ability to sit, stand, and work at a computer for extended periods Ability to communicate effectively with patients, staff, and community providers Compensation Registered Nurse (RN): $45$60 per hour Nurse Practitioner (NP): $60$80 per hour This is a temporary, part-time position with a flexible schedule between 9:30 AM and 2:00 PM, approximately 48 hours per week.
