RN MDS Assessor Role:

As a RN MDS Assessor, you will be responsible for assessing the medical, physical, and psychosocial needs of our patients.

You will:

Conduct MDS assessments to identify patient needs and develop individualized care plans Collaborate with interdisciplinary team members to ensure comprehensive care Develop and implement treatment plans that promote patient independence and well-being Monitor patient progress and adjust care plans as needed Ensure compliance with regulatory requirements and industry standards
