Job Description

At United Hospice (UH), our hospital liaisons embody a passion for making a difference in the lives of our patients and their families at a critical time when care and compassion are in the greatest need. Our Navigators are the driving force behind our hospice team, forging connections with providers, discharge planners, charge nurses and others to ensure that every patient receives the care they deserve. Salary Range For This Position:
$80,000 to $105,000. Your responsibilities include developing a hospice presence in the hospital, attending and facilitating rounds and care conferences, conferring with all health care providers to increase UH admissions.
Candidates must be adept at:
Identifying hospitalized nursing home residents who meet eligibility criteria to arrange for or conduct admissions to UH prior to discharge Must develop and maintain positive relationships with referral sources and promote exemplary customer service to physicians, facility staff, patients, and families You will serve as an integral member of our clinical team to work in conjunction with the hospital case managers in reviewing the hospital daily census to identify appropriate patients for UH admissions Admitting patients onto hospice as needed CHPN highly desirable. If not certified, must successfully sit for exam within three (3) years of the start of employment Ability to drive and daily access to a reliable automobile is required and a driver's License and driving record acceptable for agency insurance coverage are required So, what's in it for you?
