Key Responsibilities:

Perform annual home health risk assessments in patient homes Review and document medical history, current conditions, and medication usage Educate patients on their health status and proper medication management Recognize and identify care gaps, offering recommendations for improved outcomes Conduct post-discharge and annual assessments, varying per patient need No requirement for treatment planning or prescribing medications

Qualifications:

Active Nurse Practitioner license in New York Valid CPR (BLS) certification Board certification (AANP or ANCC) MSN from an accredited nursing program Minimum of 6 months recent NP experience preferred
