Key Responsibilities:

Conduct in-depth, in-home health risk assessmentsapproximately 45 minutes per visitfor Medicare Advantage plan participants Gather and evaluate medical histories, current health conditions, and medication use Provide patient education on disease management, preventative care, and safe medication practices Identify care gaps and recommend appropriate actions for follow-up or coordination Focus exclusively on assessments; no ongoing care, treatment planning, or prescribing involved

Qualifications:

Active Nurse Practitioner license in the state of NY Board Certified through AANP or ANCC Current BLS CPR certification Master of Science in Nursing (MSN) from an accredited program Minimum of 6 months recent Nurse Practitioner experience preferred Strong interpersonal and time management skills Enjoy competitive pay of $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
