Registered Nurse (RN) - Niagara County - Part Time w/ Benefits
Job Description
Liberty Home Care is looking for a motivated Registered Nurse to join our Licensed Home Care team as an RN Field Supervisor! This Part Time opportunity offers great benefits and a consistent M-F 9a-2p
•25 hour per week schedule.
Schedule:
9am
•2pm Monday
•Friday (25 hours per week)
Service Area:
Niagara County Duties & Responsibilities:
Provide assessment of newly referred patients and assess health needs in home care setting Work with patient to create a patient centered plan of care Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family Prepare written documentation and summaries of the patient's progress as needed for the continuation of home health services Provide supervision and training of PCA/HHA staff
Qualifications:
NYS Licensed Registered Professional Nurse Minimum of Associate's Degree required One year Nursing experience required Ability to travel throughout Niagara County for home based visits Home care experience a plus
