Key Responsibilities:

Conduct home health risk assessments, averaging 45 minutes per visit Evaluate patient medical histories, current conditions, and medication regimens Educate patients on diagnoses, medication use, and overall wellness Identify gaps in care and recommend steps for closure No development of treatment plans or medication prescribing required

Qualifications:

Active Nurse Practitioner license in New York MSN degree from an accredited nursing school Current Board Certification through AANP or

ANCC BLS/CPR
