Direct Hire Salary:
$85,000•$90,000 annually, based on experience
Schedule:
Monday-Friday | 9:00 AM•5:00 PM Occasional earlier start times as needed No evenings or overnight shifts This role is open exclusively to New York State-licensed Registered Nurses (RN) .
Job Overview:
We are seeking a compassionate and organized Registered Nurse (RN) Case Manager to provide healthcare oversight and coordination for individuals with intellectual and developmental disabilities in residential settings throughout the Hudson Valley. This role focuses on nursing assessments, care coordination, and collaboration with interdisciplinary teams, offering an excellent opportunity for RNs interested in community-based care rather than a hospital setting.
RN Responsibilities:
Coordinate and oversee healthcare services for individuals in group homes Perform nursing assessments and clinical evaluations Monitor overall health status and ensure continuity of care Collaborate with interdisciplinary teams to support positive health outcomes Schedule and manage care across assigned residential locations Maintain accurate documentation and health records Respond to changes in patient conditions and coordinate follow-up care
Qualifications & Requirements:
Active New York State Registered Nurse (RN) license required Bachelor's degree preferred Prior nursing experience required Experience working with developmental disabilities or community care is a plus Valid driver's license and reliable transportation required Strong communication, organization, and care coordination skills Ability to work independently in a field-based setting Why Join This Opportunity?
