Job Description
1:1 Personal Care Nurse Position Summary We are seeking a compassionate, dependable, and skilled Personal Care Nurse to provide one-on-one nursing care for an individual in their home or other designated setting. The nurse will deliver personalized, high-quality care while promoting the client's safety, comfort, dignity, and independence. This role requires excellent clinical judgment, strong communication skills, and the ability to build a trusting relationship with the client and their family. Key Responsibilities Provide individualized nursing care according to the client's care plan and physician's orders. Assess, monitor, and document the client's physical and emotional condition. Administer medications, treatments, and other prescribed therapies safely and accurately. Monitor vital signs and identify changes in the client's condition. Assist with activities of daily living (ADLs), including mobility and feeding, as needed. Assist with mobility, transfers, and fall prevention while maintaining client safety. Manage medical equipment such as oxygen therapy, feeding tubes, tracheostomy care, ventilators, or catheters, if applicable. Maintain accurate and timely documentation of care provided. Communicate effectively with physicians, therapists, caregivers, and family members regarding changes in the client's condition. Respond appropriately to medical emergencies and implement emergency procedures when necessary. Maintain confidentiality and comply with HIPAA and all applicable healthcare regulations. Foster a supportive, respectful, and compassionate environment that promotes the client's quality of life. Qualifications Current and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN/LVN) license in NYS. Preferred Skills Experience providing one-on-one private duty nursing. Experience caring for pediatric, or medically complex patients. Knowledge of infection prevention and control practices. Compassionate bedside manner and commitment to patient-centered care. Strong organizational and time-management skills. Physical Requirements Ability to lift, transfer, and reposition clients safely. Ability to stand, walk, bend, and perform physical tasks throughout the shift. Ability to respond quickly in emergency situations.
