Key Responsibilities:

Conduct home health risk assessments, with each visit averaging 45 minutes Perform annual health assessments or post-discharge evaluations as assigned Review medical histories, current conditions, and medication usage Educate patients on their diagnoses and prescribed treatments Identify and document potential gaps in care, recommending next steps No responsibility for prescribing medication or creating treatment plans

Qualifications:

Active Nurse Practitioner license in New York Current CPR certification (BLS) Board Certified through AANP or

ANCC MSN
