Start making a difference right away . What you'll need: Active LPN or RN License issued by NYS Minimum of 6 months experience in a home care setting. Current CPR/BLS Certification Empathy, Reliability , and a commitment to high-quality care What You'll do : Provide 1:1 nursing care and treatments according to the patient's individualized care plan. Provide G-tube and PEG tube care, including feedings, medication administration, and site care. Perform tracheostomy care and suctioning, and manage ventilator support as ordered. Monitor the patient's condition and promptly report changes to the appropriate healthcare professionals. Provide health promotion, counseling, and education to patients and caregivers. Administer medications and other prescribed interventions. Collaborate with healthcare professionals to coordinate patient care. Document patient assessments, care provided, and responses to treatment daily. Must be able to lift 50 lbs. Must be able to stand and work for extended periods. Perform additional responsibilities as required.
