Responsibilities:

Conduct in-depth home health risk assessments for Medicare Advantage plan members during annual health evaluations or post-discharge visits Assess prior and current medical history, existing conditions, and medication usage Educate patients on chronic conditions and medication regimens Identify gaps in care and offer actionable recommendations to improve patient outcomes Focus solely on assessmentno prescribing of medications or development of treatment plans required

Qualifications:

Active Nurse Practitioner (NP) license in Ohio Board Certification through AANP or ANCC required Current CPR certification (BLS) MSN degree from an accredited nursing program At least 6 months of recent NP experience preferred (newer graduates with strong clinical exposure encouraged to apply) This position offers compensation of $80$90 per evaluation, along with a mileage stipend and additional pay for administrative time.
