Step into a dynamic nursing role focused on delivering compassionate, patient-centered care in a home health setting. This full-time position offers a 13-week contract with a Monday through Friday schedule from 8:00 AM to 5:00 PM. Youll manage a caseload of approximately 35 patient visits per day (targeting 30 points per week) and provide essential services directly in patients homes within your assigned territory.

Key Qualifications:

Active RN license (in good standing) for the state of practice At least 1 year of recent experience in Home Health, with field experience strongly preferred Proficiency with Home Care Home Base (HCHB) electronic documentation system Experience conducting OASIS assessments Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

Complete thorough OASIS assessments and create personalized care plans for each patient Deliver skilled nursing care, such as wound care, medication administration, IV therapy, and chronic disease management Monitor patient progress, adjust care as needed, and communicate changes to the care team and physicians Educate patients and families on disease processes, medications, and home care strategies Maintain accurate, real-time documentation in HCHB Collaborate with therapists, social workers, and home health aides to coordinate comprehensive care Participate in case conferences, team meetings, and relevant continuing education activities Uphold regulatory and agency standards in all aspects of clinical documentation and service provision
