Job Description

Summary The Quality Assurance position is responsible for supporting the Home Health Division through ongoing monitoring, auditing, education, and performance improvement activities to ensure compliance with regulatory requirements, agency policies, and standards of quality care. Key Responsibilities Conduct clinical documentation reviews to support accuracy, timeliness, quality, and regulatory compliance. Reviews start of care, resumption of care, recertification. Discharge, and other key clinical documentation for quality, consistency, and regulatory compliance. Complete and /or review ICD-10 coding to support accurate diagnosis assignment, appropriate reimbursement, and compliance with Medicare home health requirements. Review, approve, and sign off on plans of care to ensure they are accurate, complete, patient-centered, and aligned with physician orders Support QAPI activities, performance improvement projects, audit tracking, and quality outcome monitoring. Provide education, coaching, and feedback to clinical staff related to documentation standards, regulatory expectations, and best practices Assist with survey readiness, policy and workflow updates, corrective action follow-up and ongoing compliance monitoring. Participate in meetings and collaborate with interdisciplinary team members to promote quality outcomes, patient safety and continuous improvement Maintain confidentiality of patient, staff, and organizational information in accordance with HIPPA Perform other duties as assigned to support home health department.
Job Requirements:
Current RN License preferred or required, depending on the scope of review. Home Health experience and ICD 10 Coding experience preferred. Current RN License preferred or required, depending on the scope of review. Knowledge of Medicare home health regulations, OASIS, care planning, documentationstandards, and QAPI processed. Strong attention to detail, organization, communication, and critical thinking skills. Ability to provide constructive feedback and support staff education in a professional manner. Certificates, Licenses, Registrations Must be a Registered Nurse in the State of Ohio. Must have a valid driver's license with an acceptable driving record and proof of insurance.
