Schedule:

Monday Friday, 8:00 AM 5:00 PM Full-time, 13-week contract

Key Responsibilities:

Conduct comprehensive assessments (OASIS) and develop individualized care plans Deliver skilled nursing services such as wound care, IV therapy, medication administration, and management of chronic illness Closely monitor patient progress and communicate updates with physicians and the care team Educate patients and families on health management, medications, and preventive strategies Complete timely documentation using Home Care Home Base (HCHB) Collaborate with therapists, social workers, and aides to ensure a coordinated approach to patient care Attend case conferences and staff meetings as needed Follow agency and regulatory guidelines for documentation and service

Requirements:

Active RN license in the state of practice, in good standing Minimum of 1 year recent field-based Home Health RN experience Familiarity with Home Care Home Base (HCHB) and OASIS required Reliable transportation, valid drivers license, and current auto insurance Strong clinical assessment, communication, and organizational skills Ability to work independently and effectively manage a flexible, field-based caseload BSN and prior weekly case management experience (2532 points) preferred Knowledge of Medicare/home health standards a plus
