Job Overview Join our dedicated hospice care team as a Hospice RN Weekend Warrior, where your expertise and compassionate care will make a meaningful difference in patients' lives during weekends. This energetic role offers an opportunity to provide high-quality end-of-life care, manage complex medical needs, and support families with empathy and professionalism. As a vital part of our team, you will deliver comprehensive nursing services in patients' homes or care facilities, ensuring comfort and dignity at every stage. This position is perfect for experienced registered nurses seeking a flexible weekend schedule while making a profound impact on patient well-being. Duties Provide expert nursing assessments, including vital signs, symptom management, and medication administration, focusing on acute pain management and palliative needs Collaborate with interdisciplinary teams to develop and implement personalized care plans aligned with hospice standards and NCQA (National Committee for Quality Assurance) guidelines Administer treatments such as IV infusions, catheter care, ostomy management, and wound care using aseptic techniques to ensure patient safety Educate patients and families about disease progression, medication use (including botulinum toxin treatment when applicable), and comfort measures to enhance quality of life Document all patient interactions accurately within Electronic Health Records (EHR) systems like Epic or Cerner, maintaining HIPAA compliance and medical records integrity Triage patient symptoms over the phone or in person to determine urgency and appropriate level of care, utilizing triage protocols for emergency response procedures Support patients with complex needs such as dementia or Alzheimer's care, ensuring safety in memory or behavioral health scenarios Experience Registered Nurse (RN) license with current state certification required; experience in hospice & palliative medicine preferred Critical care experience including ICU or trauma center background highly desirable for managing acute situations effectively Proven expertise in home care settings, including long-term care, assisted living, or nursing home environments Familiarity with EMR/EHR systems such as Athenahealth, eClinicalWorks, or similar platforms for seamless documentation Hands-on experience with advanced procedures like suturing, tube feeding management, ventilator support, and laboratory specimen collection/processing Knowledge of ICD coding (ICD-9/ICD-10), CPT coding, and discharge planning processes to ensure accurate documentation and billing compliance Skilled in pain management techniques and medication administration tailored to hospice patients' needs Experience working with diverse populations including individuals with disabilities or developmental challenges; familiarity with trauma medicine or infectious disease care is a plus Join us as a Hospice RN Weekend Warrior to deliver compassionate end-of-life care that truly transforms lives.
