We are seeking a Registered Nurse! Ross and Athens County, OH Join our Team! Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We meet people in their homes and communities and help connect them to the resources. We serve Southeastern and Central Ohio with a comprehensive array of behavioral health and other services, working with local partners to promote healthy people and strong communities. Our services are intended to be collaborative and personalized for the individual. The Registered Nurse is a member of the multidisciplinary Assertive Community Treatment (ACT) team and provides intensive, community-based services to individuals with serious mental illness and complex behavioral health needs. The Registered Nurse supports psychiatric treatment, medication management, physical healthcare coordination, health education, and Community Psychiatric Supportive Treatment (CPST) as part of the team's shared caseload. During the development of ISBH's ACT services, this position will work across Ross and Athens Counties until dedicated ACT teams are fully established. The position's primary team assignment and service area may evolve as county-based teams are established and ACT services expand into additional counties. Services are delivered primarily in individuals' homes and other community settings, with additional work occurring in office, clinic, telehealth, and hospital settings as appropriate. The Registered Nurse works collaboratively with ACT team members, healthcare providers, hospitals, pharmacies, community partners, individuals served, and their natural supports to promote recovery, community stability, and reduced reliance on emergency and inpatient care.

Essential Functions:

• Provides nursing services as part of a multidisciplinary ACT team serving a shared caseload.
• Initially travels throughout Ross and Athens Counties to support developing ACT teams and services. Travel requirements and team assignments may change as dedicated teams are established and services expand into additional counties.
• Delivers services in individuals' homes, community settings, offices, clinics, hospitals, and other locations based on individual need.
• Participates in daily ACT team meetings, service planning, care coordination, and assignment of needed services.
• Completes nursing assessments and monitors individuals' psychiatric symptoms, physical health conditions, medication response, side effects, and overall functioning.
• Supports psychiatric treatment and medication management in collaboration with ACT prescribers and other members of the treatment team.
• Administers medications, including long-acting injectable medications, within the scope of nursing practice and organizational policy.
• Provides medication education, adherence support, medication reconciliation, and coordination with pharmacies and prescribing providers.
• Coordinates laboratory testing, primary care, specialty medical care, hospital follow-up, preventive care, and treatment for co-occurring health conditions.
• Provides health education and supports individuals in developing skills to manage chronic medical and behavioral health conditions.
• Delivers medically necessary CPST and other authorized ACT services that are person-centered, recovery-oriented, problem-focused, and responsive to individual, family, and community strengths.
• Assists with crisis assessment, intervention, safety planning, and coordination with hospitals, crisis services, and other community resources.
• Participates in hospital admission and discharge planning and supports transitions between inpatient, residential, and community-based care.
• Engages individuals who may have difficulty participating in traditional office-based treatment through assertive outreach and relationship-based care.
• Collaborates with family members, natural supports, guardians, healthcare providers, and community partners when clinically appropriate and properly authorized.
• Maintains timely, accurate, and complete clinical documentation in the electronic health record.
• Meets applicable service, documentation, billing, and productivity requirements.
• Supports ACT fidelity, quality improvement, regulatory compliance, and program evaluation activities.
• Maintains knowledge of available medical, behavioral health, housing, benefits, crisis, and other community resources within the assigned service area.
• Participates in rotating after-hours, weekend, or crisis coverage when required by program operations.
• Performs other duties as assigned.

Minimum Requirements:

Education/Licensure:

• High school degree or GED equivalent is required
• Associate's Degree or Bachelor's in nursing required
• Current registered nursing license in the state of Ohio is required

Experience:

• Two years of nursing experience within the medical or behavioral health field required
• Previous experience in psychiatric medicine or Psychiatric-Mental Health Nursing Certification is preferred

Knowledge, Skills and Abilities:

• Excellent communication skills, both oral and written is required
• Broad knowledge of community service systems is preferred
• Willingness to participate in and lead collaborative community building activities is required
• Knowledge of serious mental illness, co-occurring substance use disorders, psychiatric medications, chronic health conditions, and recovery-oriented care is required.
• Ability to work effectively as part of a multidisciplinary team with a shared caseload is required.
• Ability to provide services independently in homes and community settings while maintaining appropriate communication with the ACT team is required.
• Ability to use an electronic health record (EHR) is required
• Excellent organizational skills are required
• Ability to bill Medicaid without restrictions is required
• A valid driver's license and the ability to operate a motor vehicle is required
• An appropriate level of auto insurance coverage is required
• Ability to manage deadlines within a fast-paced, high-volume environment is required
• Ability to operate in an Internet-based, automated office environment is required
• Ability to maintain high-speed internet connection is required

Physical Requirements:

• Prolonged periods of sitting at a desk and working on a computer is required
