A per diem opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments in Allen County, Ohio. This role involves traveling to members residences in a variety of communities, including Lima and surrounding townships, to evaluate Medicare Advantage plan members. You will make a vital difference for patients by assessing health status and engaging in meaningful patient educationall without the responsibility for prescribing medications or developing treatment plans.

Key Responsibilities:

Complete in-home health risk assessments, lasting approximately 45 minutes per visit Review and evaluate current and past medical history, ongoing conditions, and medication use Deliver clear patient education regarding medical issues and medications Identify gaps in care, offering actionable recommendations for resolution Visits may include annual health assessments as well as post-discharge follow-ups

Qualifications:

Active and unrestricted Nurse Practitioner license in Ohio Board-certified through AANP or ANCC Minimum of a Master of Science in Nursing (MSN) from an accredited institution Current CPR (BLS) certification required At least 6 months of nurse practitioner experience, preferably in a home health environment This per assessment role offers a competitive rate of $80-$90 per evaluation, with additional mileage and administrative stipends provided.
