Key Responsibilities:

Perform comprehensive annual and post-discharge home health risk assessments on Medicare Advantage members Review patients past and current medical histories, existing conditions, and medication regimens Educate individuals on the management of their health conditions and medications Identify care gaps and suggest recommendations to improve patient outcomes Focus solely on assessmentno requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Ohio Current CPR certification (BLS) Board certification from AANP or

ANCC MSN

degree from an accredited nursing program Minimum of 6 months NP experience preferred
