A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

What You'll Do:
Perform comprehensive start-of-care, recertification, resumption, and discharge assessments Provide skilled nursing visits including assessments, medication management, wound care, disease management, and patient education Coordinate care with physicians, interdisciplinary team members, patients, and caregivers Accurately complete OASIS documentation and maintain timely, compliant clinical records Monitor patient progress and revise care plans as needed Promote patient safety, independence, and optimal health outcomes Adhere to all Medicare, state, and agency regulations and standards
What You'll Need:
Current RN license in the state of Ohio Minimum of 1 year RN experience (home health experience preferred) Solid understanding of Medicare/Medicaid guidelines as it applies to Home Health Care Experience with OASIS documentation, preferred Excellent communication, time management, and organizational skills Valid driver's license, reliable transportation, and current auto insurance Attend meetings, CEUs in the North Olmsted, Ohio office on a regular basis
