Take your nursing expertise into the community as a field-based RN, delivering vital care directly to patients' homes. This is a full-time, 13-week contract with a weekday schedule that offers both professional fulfillment and work-life balance.

Monday to Friday, 8:00 AM 5:00

PM Caseload:

30 points per week (typically 35 patient visits per day) In this role, you will: Travel within your assigned territory to provide attentive home health care (mileage reimbursement included). Conduct comprehensive OASIS assessments and develop personalized patient care plans. Administer skilled nursing care such as wound care, medication management, IV therapy, and chronic disease support. Closely monitor and evaluate patient progress, proactively identifying any changes in condition. Collaborate with physicians, physical therapists, occupational therapists, social workers, and home health aides to ensure integrated care delivery. Educate patients and their families on disease management, treatment plans, medication protocols, and preventive strategies. Complete timely and accurate documentation using Home Care Home Base (HCHB). Actively participate in case conferences, staff meetings, and continuing education opportunities. Ensure compliance with all regulatory requirements and clinical standards.

Ideal candidates will have:
