Job Description
Overview Join our dynamic healthcare team as a Nurse Practitioner (CNP/APRN) specializing in Nursing Home Care! In this vital role, you will provide comprehensive, patient-centered care to residents in long-term care settings, ensuring their health, comfort, and quality of life. Your expertise will support interdisciplinary teams by delivering advanced clinical assessments, managing chronic conditions, and implementing treatment plans tailored to each individual's needs. This position offers an exciting opportunity to make a meaningful difference in the lives of seniors and individuals requiring specialized nursing home care. Responsibilities Provide routine and urgent medical care to nursing home residents Complete physical exams and health assessments Diagnose and treat acute and chronic health conditions Prescribe medications and other treatments See residents after they return from the hospital to help them recover safely Respond to changes in a resident's condition and treat problems early when possible Help prevent unnecessary emergency room visits and hospital stays Work with physicians, specialists, therapists, and nursing staff to coordinate care Educate residents, families, and facility staff about medical conditions and treatment plans Keep accurate and timely medical documentation Participate in care planning and quality improvement activities Requirements Active Ohio Certified Nurse Practitioner (CNP/APRN) license Current prescriptive authority and DEA license (or ability to obtain) At least 3 years of clinical healthcare experience Strong assessment and communication skills Ability to work independently and as part of a team Preferred Experience in one or more of the following: Long-term care Skilled nursing Geriatrics Primary care Managed care Post-acute care Why You'll Love Working Here Monday through Friday schedule Flexible 8-hour workday Great work/life balance Visit 1-2 nursing facilities each day Build lasting relationships with your patients Work with a supportive team that values your experience and ideas Make a direct impact by helping residents stay healthier and avoid unnecessary hospital visits Join us in providing compassionate, expert care that enhances the well-being of our residents!
