Full Job Description Hospice Registered Nurse Case Manager Salary:
$80-89k/yrBOE Schedule:
Monday-Friday| 8:00 AM- 5:00PM Location:
West/Franklin County, Ohio, and surrounding areas, including Grove City and Hilliard Requirements of the RNCM role: Licensed Registered Nurse BLS Certified Comfortable working in both private homes and facility settings. Valid Driver's License and Auto Insurance Coverage Reliable transportation A minimum of 1 year of Clinical experience Hospice experience preferred About the RNCM role: Conduct comprehensive patient assessments, including vital signs and medical history review Travel to patients' residences to conduct visits Develop individualized care plans in collaboration with multidisciplinary teams Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met. Perform evaluations for hospice care along with informational visits as needed. Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care. Educate patients and families on treatment plans, medication management, and health maintenance strategies Submit complete, accurate, and relevant clinical notes promptly. Participate in IDG and case conferences Conducts an initial and ongoing comprehensive assessment of the patients Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care. Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed. Provide care plan to Hospice Aide and LPN as part of the IDG team.
