Weekend On-Call Registered Nurse After-hours support. Mostly remote. Critical impact. As a Weekend On-Call Registered Nurse, you will provide skilled nursing support, symptom management, and crisis intervention to hospice patients receiving care in homes and facilities throughout the Cleveland area. Key Responsibilities Respond to after-hours and weekend patient, family, and caregiver needs by providing timely nursing assessments, interventions, and clinical support. Perform on-call visits to assess changes in patient condition, manage symptoms, and implement appropriate care plans. Coordinate care with physicians, caregivers, pharmacies, and interdisciplinary team members to ensure continuity of care. Provide education and guidance to patients, families, and caregivers regarding medications, symptom management, and end-of-life care. Facilitate admissions, deaths, and other urgent clinical situations in accordance with hospice policies and regulatory requirements. Complete accurate and timely documentation of patient assessments, interventions, and care coordination activities in the EMR. Deliver compassionate, patient-centered care while supporting the physical, emotional, and psychosocial needs of patients and families. Qualifications Active Registered Nurse (RN) license in the state of practice required. Minimum of one (1) year of nursing experience required; hospice, home health, oncology, palliative care, or critical care experience preferred. Strong clinical assessment, symptom management, and critical thinking skills. Ability to work independently, prioritize urgent patient needs, and make sound clinical decisions in the field. Experience with electronic medical records (EMR); Homecare Homebase (HCHB) experience preferred. Excellent communication and interpersonal skills with the ability to effectively support patients, families, caregivers, and healthcare professionals. Valid driver's license, reliable transportation, and current automobile insurance required.
