Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage members Assess prior and current medical history, existing conditions, and medication use Deliver patient education on chronic conditions and prescribed medications Identify care gaps and provide recommendations for follow-up, without the responsibilities of developing treatment plans or prescribing medications Complete each risk assessment in an average of 45 minutes Visits may include annual assessments or post-discharge check-ins, depending on patient needs

Qualifications:

Current Nurse Practitioner license in Ohio required Active CPR/BLS certification Board certification via AANP or ANCC Minimum of six months recent Nurse Practitioner experience preferred Masters degree in Nursing (MSN) from an accredited program Youll receive compensation of $80$90 per evaluation, which includes a mileage stipend and administrative time, rewarding both your time and expertise.
