A full-time opportunity is available for an experienced Registered Nurse to provide home health care over a 13-week contract in a field-based setting. Youll rely on advanced assessment skills and a patient-centered approach to deliver skilled nursing care to individuals in their own homes, ensuring high quality and continuity of care.

Monday Friday, 8:00 AM 5:00

PM Key Responsibilities:

Perform comprehensive OASIS assessments and develop individualized care plans for each patient. Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management. Monitor patient progress, recognize any changes in condition, and promptly update the interdisciplinary care team. Provide education to patients and families regarding disease processes, medication regimens, and preventive care. Document all patient care activities accurately using the Home Care Home Base (HCHB) electronic system. Collaborate with physical therapists, occupational therapists, social workers, and home health aides to provide coordinated care. Attend required case conferences, staff meetings, and relevant training sessions. Ensure compliance with all regulatory documentation and agency care delivery standards. Travel daily to patient residences within your designated territory (mileage reimbursement included).

Qualifications:

Current, active RN license in good standing. At least one year of recent home health nursing experience (field work preferred). Demonstrated proficiency in Home Care Home Base (HCHB). Previous experience completing OASIS assessments. Reliable transportation, a valid drivers license, and up-to-date auto insurance.
