Key Responsibilities:

Complete annual risk assessments for members of a Medicare Advantage health plan in a home health setting Review prior and current medical histories, clinical conditions, and medication regimens during each assessment (approximately 45 minutes per visit) Educate members on their health conditions and prescribed medications Identify care gaps, recommend solutions, and document findings; no prescribing or treatment planning required Exercise sound clinical judgment while supporting holistic, individualized patient education

Qualifications:

Current Nurse Practitioner license in Ohio MSN from an accredited nursing program Board Certification through AANP or ANCC Active BLS (CPR) certification At least 6 months of recent NP experience preferred This role offers $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
