Deliver patient-centered care and make a meaningful difference as a Home Health Registered Nurse (RN)! We are searching for an experienced RN with a passion for clinical excellence and the ability to thrive in an independent field environment. Youll work one-on-one with patients in their homes, fostering positive outcomes through skilled nursing services, education, and health advocacy.

Schedule:

Monday Friday, 8:00 AM 5:00 PM Full-time, 13-week contract

Key Duties:

Complete OASIS assessments and develop tailored care plans Deliver comprehensive nursing care, including wound care, medication administration, IV therapy, and chronic disease management Monitor patient progress, respond to changes in condition, and maintain frequent communication with care teams and physicians Educate patients and families on treatment plans, medications, and self-care techniques Document care and assessments accurately in Home Care Home Base (HCHB) system Collaborate with therapists and social workers to promote coordinated care Participate in case conferences, staff meetings, and ongoing education

Qualifications:

Active, unencumbered RN license in Ohio At least 1 year of recent field-based Home Health experience Proficiency with Home Care Home Base (HCHB) and OASIS required Strong clinical assessment skills and sound judgment Reliable transportation, valid drivers license, and adequate auto insurance Excellent independence, organization, and communication

Preferred:

Bachelor of Science in Nursing (BSN) Experience with 2532 points/week caseload Familiarity with Medicare and home health regulatory requirements
