$53-$ 55.90/hour

Mileage:

Reimbursed between patient visits

Contract:

Two to Three Month contract

Schedule:

Monday-Friday, 8:00 AM-4:30 PM

On-Call/Weekend:

One week of on-call coverage, including a weekend

Coverage Area:

Lake, Geauga, and Ashtabula Counties Responsibilities

• Manage a home health patient caseload throughout the assigned service area.
• Assess patient needs and coordinate individualized plans of care.
• Monitor patient progress and communicate changes in condition to physicians and the healthcare team.
• Educate patients and caregivers on treatment plans, medications, and disease management.
• Maintain accurate and timely clinical documentation.
• Travel between patient visits throughout the coverage area.
• Participate in the required on-call rotation. Requirements
• Active Ohio Registered Nurse (RN) license.
• Ability to work independently and manage a patient caseload.
• Valid driver's license and reliable transportation.
