Responsibilities Conduct comprehensive patient assessments to determine care needs and develop individualized care plans. Manage patient care across various settings including hospice, home health, outpatient clinics, and inpatient facilities. Collaborate closely with healthcare professionals such as physicians, social workers, therapists, and case managers to coordinate treatment plans and ensure seamless patient care. Perform clinical case management tasks including discharge planning, medical records documentation, and utilization management. Utilize electronic health records (EHR) systems to accurately document patient information, progress notes, and treatment updates while maintaining HIPAA compliance. Monitor vital signs, assess physiological status, and evaluate diagnostic results to inform ongoing patient care strategies. Provide education and support to patients and families regarding disease management, medication administration, and end-of-life care options in hospice settings. Skills Critical care experience with a strong understanding of physiology and complex patient conditions. Experience working in Level I or Level II trauma centers, emergency rooms, ICU units, or hospital medicine environments. Knowledge of infectious disease protocols, post-acute care procedures, and hospital regulation policies. Skilled in managing diverse patient populations including geriatrics, pediatrics (PICU), behavioral health patients, and those requiring acute or outpatient care. Proficiency in medical terminology, EMR/EHR systems, discharge planning, and medical records management. Excellent patient service skills with the ability to communicate effectively with clients and collaborate with healthcare teams. Familiarity with managed care practices, utilization management processes, diagnostic evaluation techniques, and HIPAA regulations for privacy protection.
