Job Summary We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice and home care team. In this vital role, you will provide high-quality, patient-centered nursing care to individuals in their homes or hospice settings. Your expertise will help improve quality of life for patients facing serious illnesses by delivering comprehensive assessments, managing symptoms, and coordinating care plans. Responsibilities Conduct thorough patient assessments, including vital signs, physiology evaluation, and symptom management to develop personalized care plans. Provide direct patient care in home or hospice settings, ensuring comfort and dignity for individuals with complex health needs. Collaborate effectively with healthcare professionals such as physicians, social workers, and therapists to coordinate comprehensive care strategies. Manage electronic health records (EHR/EHR), medical records, and documentation in compliance with HIPAA regulations and health policy standards. Assist with discharge planning, medical-surgical procedures, and post-acute care transitions to facilitate smooth patient recovery or end-of-life care. Perform diagnostic evaluations, manage case management activities, and oversee patient service delivery including intake procedures. Educate patients and families about disease processes, medication management, and home health protocols to promote safety and understanding. Work with homecare/hospice team in the office to ensure a "Unique" care plan for both home care and hospice. Experience Proven experience in hospice or home health nursing with a strong background in critical care, ICU, emergency medicine, or medical-surgical environments. Demonstrated expertise in managing patients with infectious diseases, behavioral health issues, geriatrics, pediatrics, or post-acute conditions. Familiarity with hospital medicine practices such as discharge planning, utilization management, and clinical case management. Knowledge of health regulation policies including HIPAA compliance and medical terminology essential for accurate documentation. Experience working within managed care settings or outpatient clinics is highly desirable. Strong skills in patient assessment, vital signs monitoring, diagnostic evaluation, and managing complex cases across inpatient and outpatient environments. Ability to collaborate seamlessly with multidisciplinary teams while maintaining excellent client communication skills.
