Schedule:

13-week contract, with potential extension Monday through Friday (full-time, on-call probable) Structured daily visit expectations

Core Responsibilities:

Deliver skilled in-home nursing care, including assessments and interventions Complete OASIS documentation accurately and promptly for each patient Collaborate closely with physicians and interdisciplinary teams to develop and execute individualized care plans Perform wound care, medication administration, patient education, and chronic disease monitoring Communicate relevant updates and changes in patient conditions to the clinical team

Qualifications:

Active, unrestricted RN license Minimum 1 year of recent home health RN experience Demonstrated proficiency with OASIS documentation Strongly preferred: Experience with HomeCare HomeBase software Proven ability to manage daily home visits and maintain organized scheduling Valid drivers license and reliable transportation
