Schedule:

13-week contract, potential to extend Full-time, Monday through Friday On-call participation possible Daily, structured patient visit requirements

Qualifications:

Current and active RN license At least 1 year of recent home health experience as an RN Strong proficiency with OASIS documentation HomeCare HomeBase experience preferred Proven ability to manage a structured schedule and daily patient assignments Reliable transportation and valid drivers license

Key Responsibilities:

Deliver compassionate nursing care in patients homes, including skilled and direct interventions Complete accurate, timely OASIS assessments for each patient visit Partner with physicians and interdisciplinary teams to develop and implement individualized care plans Provide wound management, medication administration, disease monitoring, and patient/caregiver education Promptly communicate patient status updates and any changes in condition to the clinical care team
