Weekly Pay, Vision, Dental, Medical Position OverviewOhio's Choice Home Health is seeking a compassionate and experienced Registered Nurse (RN) to provide skilled home health services to patients throughout Mahoning, Columbiana, Trumbull, and portions of Portage County. This role is ideal for a clinician who thrives in an independent environment, provides exceptional patient care, and values strong communication and collaboration with interdisciplinary team members.
Schedule & Compensation DetailsWeekly Hour Guarantee:
36 hours
Schedule:
Monday through Friday, 8:00 AM - 4:30
PMOn-Call Requirement:
2-3 times per week
Weekend Requirement:
Every other weekend (Saturday and Sunday) with on-call responsibilities
Mileage Reimbursement:
Yes, reimbursement is provided between patient visitsCoverage Area Key ResponsibilitiesDeliver high-quality skilled nursing care in patients' homes.

Complete comprehensive patient assessments and ongoing evaluations.

Develop, implement, and update individualized plans of care.

Accurately document all patient visits and care provided.

Meet established productivity expectations while maintaining quality outcomes.

Participate in weekly case conferences and team meetings.

Provide rotating on-call coverage and weekend support as assigned.

Maintain frequent communication with clinical staff, physicians, and office personnel to ensure continuity of care.

Educate patients and caregivers regarding disease management, medications, and treatment plans. Required QualificationsActive and unrestricted Registered Nurse (RN) license.

Current clinical nursing skills and competencies.

Ability to work independently while managing a home health patient caseload.

Strong documentation, communication, and organizational skills.

Reliable transportation and valid driver's license. Preferred QualificationsPrevious home health experience. Experience with Homecare Homebase (HCHB) documentation software. Strong clinical assessment and care coordination skills. Ability to adapt to a fast-paced, patient-focused environment. Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
