Key Responsibilities:

Conduct comprehensive annual health risk assessments with Medicare Advantage members in their homes Review and evaluate current and previous medical and medication histories Educate patients on medical conditions, preventive care, and medication management Identify potential gaps in care; offer recommendations to enhance outcomes Focus solely on assessmentno requirement for treatment planning or prescribing medications

Qualifications:

Active Nurse Practitioner license in Ohio Board Certification via AANP or ANCC Current BLS certification (CPR) MSN from an accredited institution At least 6 months of recent Nurse Practitioner experience preferred
